ACTIVITIES & RISKS

I am the parent or legal guardian of the child(ren) listed above. I request permission for them to participate in Fostoberfest Kids Zone activities, which may include bounce houses, inflatable slides, games, and related activities. I understand these activities involve risks, including slipping, falling, jumping or landing improperly, collisions with people or equipment, and injuries ranging from cuts and sprains to broken bones, head injuries, or other serious harm. I understand that supervision and safety measures cannot eliminate every risk.

SAFETY RULES & RESPONSIBILITY

I will ensure each child follows posted rules and staff instructions, including age, height, capacity, and activity restrictions. I will not permit participation if a child is ill, injured, or unable to participate safely. Staff may refuse or stop participation for safety reasons.

PARENT/GUARDIAN SUPERVISION REQUIRED

I must stay with and actively supervise my child(ren) at all times in the Kids Zone. Children may not be left unattended. Event staff, volunteers, and inflatable attendants operate activities and enforce safety rules; they do not provide childcare or individual supervision. I am responsible for my child(ren)'s behavior and compliance with instructions. Failure to supervise may result in removal from activities without refund.

ASSUMPTION OF RISK & RELEASE

To the fullest extent permitted by California law, on behalf of myself and, to the extent legally permitted, my child(ren), I knowingly assume the inherent risks of participation and release the Foster City Chamber of Commerce, Foster City Chamber Foundation, and their respective directors, officers, employees, volunteers, sponsors, and agents from claims for injury, loss, or damage arising from participation, including claims based on ordinary negligence, except to the extent such claims cannot lawfully be released. This release does not apply to gross negligence, reckless misconduct, or intentional wrongdoing. I understand that California law may limit the enforceability of certain releases involving minors.

EMERGENCY CARE

If I cannot be reached promptly, I authorize event personnel to contact emergency services and seek reasonably necessary emergency medical assistance for my child(ren). I understand I am responsible for medical expenses to the extent permitted by law.
